 A Brief Overview

The position provides medical nutrition therapy, diabetes self-management education to University Hospitals clients and clients within the surrounding communities.

What You Will Do
  • Conduct individualized needs assessment with the participation of the patient, family and support systems.
  • Apply principles of teaching and learning and/or behavior change to facilitate self-management skills of individuals with diabetes and chronic kidney disease.
  • Collaborate with a multidisciplinary team that includes the patient, certified diabetes educator and physicians to support patient success with behavior change and euglycemia.
  • Establish and maintain relationships with diverse populations.
  • Acts as a key resource for diabetes education for the hospitals physician office groups.
  • Maintains data base and tracking systems of patients requiring diabetes education and applies appropriate metrics to determine outcomes.
  • Participate in case management of patient included, but not limited to, discharge planning, chart review, assessment, telephonic outreach, appointment scheduling, home or doctor visits and coordination of care with other departments.
Additional Responsibilities
  • Adheres to department standards regarding acceptable attendance, reporting to work on time and completing work in designated time.
  • Attends all mandatory department meetings.
  • Performs other duties as assigned.
  • Complies with all policies and standards.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
 

Qualifications

 Education
  • Bachelor's Degree in Nursing or Dietetics (Required)
Work Experience
  • 3+ years experience (Required)
Knowledge, Skills, & Abilities
  • Microsoft Office (Preferred proficiency)
  • EPIC (Required proficiency)
  • Basic computer literacy (Required proficiency)
  • Highly effective oral and written communication skills (Required proficiency)
  • Demonstrated leadership skills (Required proficiency)
  • Good problem-solving and decision-making skills (Required proficiency)
Licenses and Certifications
  • Basic Life Support (BLS) (Required)
  • Certified Diabetes Educator (CDE) (Required within 1 Year)
