We also provide medication assistance support to ensure our patients have access to the lowest cost for their medications, transportation assistance to and from appointments if needed, community resource connections to close gaps with social determinants of care, integrated care models including targeted chronic disease management support with our pharmacy team and much more. We assist patients in getting access to medicaid as well as other insurance options using trained professional staff who serve in these roles.
Hours of operation: Monday-Friday 7:30AM-6:00PM, Saturdays 8:00AM-12:00PMPharmacy: Monday-Friday 8:30AM-6:00PMThe Medical Records & Referral Specialist works as part of the clinical support team and serves a vital role in maintaining continuity of care for health center patients.
Duties include
· Recording information in the patient’s electronic medical record in a concise and timely manner so that accurate reports may be retrieved regarding the care and the entire referral management process, including tracking patient compliance with referral completion.
· Assisting patients in identifying and navigating the healthcare system in order to support completion of all referrals.
· Direct patient communication and providing excellent customer service during each interaction.
· Serving as an ambassador to all current and potential referral sources on behalf of HCHC and HCHC patients.
· Medical record document management including scanning of paper documents properly into the electronic medical record; routing documents appropriately through the fax server system; preparing documents for patient pick up and other duties that support patient care needs for the health center.
· Responds to requests for release of confidential patient medical record information in accordance with applicable laws and regulations.
· Receives and responds to request for subpoenas of medical records; reviews file to determine records to be released, and responds bi-directionally regarding patient medical information releases, submits invoices for records appropriately.
· Processes ordered referrals and authorizations for patients and works closely with the clinical staff, referral specialists, employers and payers to achieve a positive impact on the outcome, cost and quality of care rendered to patients in accordance with HCHC policies, practices, procedures and applicable regulations.
· This position is authorized to work in any location recognized as within scope of HCHC by HRSA.
Requirements
· A high school diploma or equivalent is required.
· Must be able to organize patient information and handle multiple tasks concurrently.
· Possess good interpersonal skills
· Must be able to work effectively with patients and health center staff
· Must be able to work well with a culturally diverse customer base and represent HCHC in a professional manner is required.
· Working knowledge of MS office products and ability to learn other computer related applications such as electronic medical record systems is required.
· Excellent oral and written communication and interpersonal skills.
· Working knowledge of office equipment, i.e., scanners, printers, copy machines, telephone system, fax machine is required.
