Join a nationally recognized transplant team providing cutting-edge care in a dynamic and collaborative environment. We are seeking a motivated and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to support the Kidney and Pancreas Transplant Program, with cross-coverage in the Living Donor Kidney Program.

Position Summary:

The APP will provide comprehensive care to patients served by the kidney and pancreas transplantation program, including pre-transplant and living donor evaluations, post-transplant/donation management, and long-term follow-up in the outpatient setting. This role also includes collaboration across the broader transplant service, including liver and heart.

Key Responsibilities:

• Deliver care to kidney and pancreas post-transplant patients collaboratively and autonomously including immunosuppression management, order and interpretation of tests and diagnosis/management of chronic as well as acute patient problems.
• Conduct thorough evaluations and assessments of potential transplant and donor candidates. 
• Collaborate closely with multidisciplinary transplant teams, including surgeons, physicians, nurses, pharmacists, and social workers.
• Serve as an educational resource to patients and caregivers for concerns such as immunosuppressive therapy and side effects, lifestyle modifications, and signs of rejection or infection.
• Provide cross-coverage and support within the transplant program as needed.
• Participate in academic initiatives, including research, policy development, committee work, and quality improvement projects.
• Ensure accurate and timely documentation and maintain compliance with hospital policies, procedures, and regulatory standards.
• Evaluate organ offers in concert with transplant surgeons and nephrologists; coordinate transplant procedures

Qualifications:

• NYS licensure/eligibility as a Nurse Practitioner or Physician Assistant required.
• Relevant clinical experience in acute care, nephrology, or surgery preferred; candidates with medical/surgical or equivalent experience will also be considered.
• Exceptional communication, critical thinking, and organizational skills.
• Comfortable working independently and collaboratively in a high-acuity, multidisciplinary setting.
• Commitment to service excellence and continual professional growth.

Nurse Practitioner
Require a Master's degree as a Nurse Practitioner, an active New York State Registered Nurse license in good standing, an active New York State Nurse Practitioner licensure in good standing and current national certification as an Nurse Practitioner. 

Physician Assistant
Required completion of an ARC-PA approved Physician Assistant program, an active New York State Physician Assistant licensure in good standing and current national certification as a Physician Assistant by NCCPA (National Commission on Certification of Physician Assistants)

NPs and PAs will demonstrate clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn. The APP will participate in the education of learners (students and new employees), using clinical learning experiences, knowledge sharing and skill training.
