Essential Functions

• Completes professional nursing assessments to gather patient medical information, including information for the Minimum Data Set (MDS)
• Serves as a member of the Care Plan Committee
• Enters finalized care plan agreed to by all concerned parties into the computer system
• Reviews resident applications prior to admission
• Monitors and reviews resident records to ensure appropriate care plan process is being followed and Advance Directives are on file
• Participates in Quality Assurance monitoring as assigned
• Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above

Minimum Qualifications

• Requires (a) graduation from an approved nursing education program resulting in an associate OR diploma degree in nursing and two (2) years of progressively responsible professional nursing experience; OR (b) a bachelor's degree in nursing and one (1) year of professional nursing experience; OR (c) a master's degree in nursing
• Requires licensure as a Registered Nurse in the State of Illinois

Preferred Qualifications

• Two (2) years of experience displaying visual, auditory, physical, and comprehensive ability in instructing and demonstrating clinical nursing techniques and documenting information
• Two (2) years of experience evaluating patient conditions and behavior and preparing accurate, detailed reports
• Two (2) years of experience conducting assessments and documenting patient data
• One (1) year of experience as a MDS Coordinator
• Extensive knowledge and understanding of State and Federal policies, procedures, regulations guidelines

Conditions of Employment

• Current Minimum Data Sheet (MDS) Certification or attainment within six (6) months of employment
• Requires ability pass a background check
• Requires ability meet all agency vaccine/health-related policies and guidance
• Requires ability pass a drug screen and pre-employment physical
• Requires ability utilize office equipment, including personal computers
• Requires ability acquire and maintain Cardiopulmonary Resuscitation (CPR) certificate
