Inpatient Advanced Practice Provider – Nurse Practitioner or Physician Assistant

The Knight Cancer Institute is seeking inpatient Advanced Practice Providers in the Center for Hematologic Malignancies (CHM) section of the Division of Hematology & Medical Oncology.

Function/Duties of Position

To provide patient care in accordance with allied health privileges granted by OHSU.  The Advanced Practice Provider (APP) is under the supervision of the Inpatient APP Manager and provides patient care consistent with the credentials and the philosophy of the Department of Medicine, University Hospitals and Clinics.

 

The APP functions in a manner which maintains the confidentiality and privacy of each patient, promotes a positive relationship with all health team members, and supports the policies and procedures of the University Hospitals and Clinics.

 

PRACTICE DESCRIPTION AND SCOPE OF SERVICE

 

The inpatient APP is responsible for daily rounds in conjunction with the faculty physician, writing of daily notes and orders, coordinating care with the multidisciplinary team, coordinating the inpatient workflow of admissions and discharges, performing appropriate procedures (ie., bone marrow biopsies, lumbar punctures, Ommaya taps, and administration of chemotherapy), and additional duties as assigned.

 

The inpatient APP team works collaboratively in a clinical and teaching capacity with opportunities to attend and participate in relevant educational conferences and Division-specific research protocols.

 

Scope and range within unit population: Adolescent through geriatric. Cellular therapy and bone marrow transplant patients, as well as patients undergoing treatment of hematologic and oncologic malignancies.

    • Elicits patient history pertinent to current disease status and treatment.
    • Performs physical examination.
    • Analyzes patient information and laboratory data to form differential diagnoses.
    • Formulates plan of care obtaining any additional lab tests or studies, arranging return visits, providing appropriate patient education or referrals/consults for newly identified problems.
    • Arranges support services as needed in conjunction with discharge services and social work departments.
    • Participates in care conferences and discharge planning conferences.
    • Collects, prepares and submits data for review in accordance with collaborative study groups and international data registries.
  •  Emergent situations
    • Contacts attending physician to discuss emergent situations and plans.
    • Recognizes unusual or untoward reactions to conditioning or chemotherapy regimens and contacts attending physician, as appropriate.
  •  Patient education
    • Provides education for patients about their disease, treatment, and potential complications.
    • Discusses risks/benefits when prescribing medications or treatments.
  •  Patient Rights
    • Provides patient care based on the “Patient Bill of Rights.”
    • Respects patient diversity in beliefs and health care concerns.
    • Assists patient in problem solving health care concerns.
  •  Documentation of Practice
    • Records all patient visits including lab results, medication changes, current plan of care, return visits and new problems in the Electronic Health Record (EHR).
    • Documents hospital discharge summaries, procedure notes, and progress notes into the EHR in a timely manner.
