Job Title: Clinical Support Specialist (Home Health & Hospice)

 

Position Summary

The Clinical Support Specialist provides administrative and clinical coordination support to ensure the delivery of high-quality care to patients in home health and hospice settings. This role serves as a liaison between clinicians, patients, families, and office staff, supporting care planning, documentation, compliance, and day-to-day operations.

 

Reports to: Executive Director 

 

Clinical Coordination

  • Assist clinicians (RNs, LPNs, therapists, hospice staff) with care coordination and scheduling
  • Support intake and referral processes, including gathering patient information and verifying eligibility
  • Track and communicate patient care needs, updates, and changes in condition
  • Coordinate interdisciplinary team (IDT) meetings and documentation

Documentation & Compliance

  • Review clinical documentation for completeness, accuracy, and regulatory compliance
  • Ensure timely submission of visit notes, plans of care, and physician orders
  • Assist with electronic medical record (EMR) data entry, order tracking and maintenance
  • Support compliance with CMS, DPH, CHAP regulations

Patient & Family Support

  • Serve as a point of contact for patients and families regarding services and scheduling
  • Provide basic education on home health or hospice services (non-clinical guidance)
  • Facilitate communication between care teams, patients and families

Operational Support

  • Manage clinician schedules and optimize visit routes
  • Assist with supply coordination and equipment tracking
  • Support quality improvement initiatives and audits

 

Qualifications

Education & Experience

  • LPN, associate or bachelor’s degree in nursing
  • Experience in home health, hospice, or healthcare administration strongly preferred
  • Familiarity with EMR systems (e.g., Matrix Care, Homecare Homebase, Epic)

Skills & Competencies

  • Strong organizational and multitasking skills
  • Knowledge of home health and hospice workflows and regulations
  • Excellent communication and interpersonal skills
  • Attention to detail and ability to manage deadlines
  • Ability to work collaboratively in a fast-paced, interdisciplinary environment

Working Conditions

  • Office-based with frequent interaction with clinical staff
  • May require occasional on-call support or flexible scheduling
  • High attention to detail and deadlines
