Join our dedicated team at KMHS as a Residential Services Mental Health Professional, where you'll play a pivotal role in providing therapeutic psychosocial care for residents admitted to our Residential Services (RS). In collaboration with the RS team, you'll lead in all aspects of the admission and treatment process, ensuring individualized care, safety, and effective practices. Join our dedicated team at KMHS as a Residential Services Mental Health Professional, where you'll play a pivotal role in providing therapeutic psychosocial care for residents admitted to our Residential Services (RS). In collaboration with the RS team, you'll lead in all aspects of the admission and treatment process, ensuring individualized care, safety, and effective practices. Primary Responsibilities:
- Organize admission referrals, complete history reviews, and intake interviews for residents in consultation with the RS Supervisor and Recovery Case Manager.
- Collaboratively lead weekly RS clinical team meetings to optimize the resident's stay.
- Coordinate with AOP treatment teams to implement treatment plans during the resident's stay.
- Document observations, interventions, and assessments to reflect progress and relationship to care plans.
- Develop cooperative relationships with collateral contacts to support individualized residential plans.
- Monitor resident progress and collaborate on timely and supportive discharges.
- Coordinate referrals for co-occurring disorder treatment programs when necessary.
- Conduct regular group meetings and assist in crisis de-escalation and stabilization.
- Maintain and submit records and documentation to meet regulatory and funding source requirements.
- Participate in QI activities, committees, and special task forces as requested.
