About the Role:

As an LPN, you will be responsible for delivering high-quality patient care within our skilled nursing facility. You will work closely with our interdisciplinary team to develop and implement individualized care plans, administer medications, monitor vital signs, and provide education and support to patients and their families. Your experience and compassion will shine through in your ability to see patients recover and thrive under your care.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with our healthcare team

• Assist with ADLs, including bathing, dressing, and grooming

• Maintain accurate and detailed records of patient care and treatment

• Collaborate with the interdisciplinary team to develop and implement discharge plans

• Provide education and support to patients and their families on disease management, self-care, and other health-related topics
