Position Summary


Assist individuals with intellectual and developmental disabilities in their home by ensuring all health and safety needs are met with dignity.


Position Duties and Responsibilities

  1. Complete written and verbal reports/ documentation related to daily progress.
  2. Act as a positive role model to individuals served.
  3. Provide life skills support and behavior management.
  4. Assist with hygiene, grooming, assist with medication delivery, housekeeping, laundry, cooking and any other areas identified within the ISP.
  5. Provide transportation (using personal vehicle) the individual(s) served to access the community.
  6. Attend all house meetings, complete all required trainings and work within all HeartWell Policies & Procedures.

Shifts Available

Mon-Fri 7am-9am

Sat 9am-9pm, Sun 3pm-11pm

Tues & Sat 3pm-11pm

Fri 3pm-11pm, Sat 7am-3pm, Sun 9am-3pm

Sat & Sun 7am-3pm

Requirements

Position Requirements

  • Minimum – High school diploma or equivalent
  • Current and Valid Driver’s License
  • Safe insured/ registered vehicle
