About the role

We are currently seeking a Home Care RN for our partner in Urbana, IL. As a Home Care Field RN, you will join an interdisciplinary team and provide professional caregiving services. Your role will involve coordinating and delivering direct and indirect patient care using the nursing process. You will focus on meeting the physical, psychosocial, environmental, and spiritual needs of specific patient populations and their families within our partner's designated geographical area.

Responsibilities:

  • Coordinate and provide direct and indirect patient care using the nursing process
  • Meet the physical, psychosocial, environmental, and spiritual needs of specific patient populations and families
  • Comprehend department-specific indicators
  • Perform case management activities, including assessment, coordination, planning, monitoring, implementation, and evaluation
  • Interact with clients, caregivers, and families to assess needs, plan care, arrange services, monitor progress, and provide support and education
  • Complete all admission documentation within policy guidelines
  • Ensure timely completion and submission of required OASIS/485 changes
  • Document and transmit visit notes, telephone conversations, and physician orders according to policy
  • Maintain effective communication with the MD office for verbal orders and proper documentation
  • Conduct Home Health Aide supervisory visits
  • Act as the coordinator of the healthcare team to ensure seamless care delivery
  • Communicate and initiate case conferencing when necessary, documenting interactions with other disciplines, MDs, care coordinators, insurers, etc.
  • Provide care in accordance with the plan of care and orders
  • Direct the activities of licensed practical nurses
  • Assign tasks to home health aides, provide written instructions, and supervise their work in patient homes
  • Demonstrate knowledge of State and Federal regulatory and accreditation guidelines
  • Perform OASIS assessments for admissions, recertifications, resumptions, and discharges
  • Perform complex wound care, such as wound vacs, packing, and removal of staples or sutures
  • Monitor central lines, including PICC and Hickman catheters, and perform dressing changes per agency protocol
  • Access mediports for IV infusion and/or blood draws and flushes per agency protocol
  • Conduct case management for assigned patients
  • Communicate updates and changes in patient condition with MD via phone or electronic means

Requirements

  • Associate's Degree in Nursing or higher
  • Registered Professional Nurse (RN) License in Illinois upon hire
  • Basic Life Support (BLS) certification within 30 days
  • Driver's License and Proof of Auto Insurance upon hire
  • One (1) Healthcare/Medical - Nursing General qualification
  • Completion of all required continuing education regarding the care of acute stroke patients
  • Current CPR certification
  • Possession of reliable transportation, valid Illinois driver's license, and state-required insurance
