About the role

Description

PURPOSE:

To provide support and advocacy for caregivers. Assist families and team members with the foster home licensing process and the Guardianship Assistance Program. 

KEY RESPONSIBILITIES:

  • Assist families who have accepted children into their home to meet the immediate needs of the children and families.    
  • Connect families to available resources in the community.
  • Educate the families on Level 1 Licensing and the funding and benefits that are available.   
  • Attend Multi-Disciplinary Team staffing's and complete service referrals.
  • Coordinate resolutions with Case Management and/or Child Protective Investigator team to solve any barriers with required documentation. 
  • Maintain accurate and timely documentation of spreadsheets and materials. 
  • Assist with GAP redeterminations and other GAP duties as requested.
  • Other duties as assigned to include administrative tasks as needed.

Requirements

EDUCATION / EXPERIENCE:

Bachelor’s degree in social services or related filed preferred. Experience in social services/child welfare may substitute the degree requirement.
