Title: Associate II, Population Health - Specialty

Role Description:

The ideal candidate will have strong written and verbal communication skills, extremely detail-oriented, comfortable taking ownership & following up, be self-directed, and enjoy working with a team in a fast-paced startup environment, while bringing their experience and innovative ideas. The content of this role with focus on Specialty Care at Oak Street.

Core Responsibilities:

• Development, testing, and troubleshooting all field facing tools / data related to Specialty Care at Oak Street, including but not limited to our integration with RubiconMD

• Partner with Canopy product owners on development, design, functionality, and reporting needs as it relates to Specialty Care

• Drafting proposals for new logic / tool functionality and illicit feedback / input from various cohorts including but not limited to PHDs, Sr. CISs, Clinical Coaches / Medical Directors

• Establish communication cadence / content for regional leaders regarding Specialty Care initiatives

• Creation of metrics and reports to monitor success and identify opportunities for improvement

• Identify and implement opportunities to improve our Specialty Care initiatives

• Other duties as assigned
