Responsibilities

In collaboration with physicians, residents, patients and families, the RN-Clinician II facilitates the delivery of quality, evidenced-based care, while promoting efficient and effective patient outcomes. This position provides education and support to physicians, staff, patients, and families regarding innovations/evidence for quality outcomes. In collaboration with patient and families, the RN-Clinician II shares information related to the plan-of-care and facilitates communication among patients/family and all care providers. Other responsibilities include using information technology to obtain data for use in the clinical setting to assess compliance with unit, hospital and national quality goals; assisting with ensuring compliance with disease-related measurement such as core measures for Congestive Heart Failure, and safety bundles for anticoagulant therapy or Infection prevention; and applying knowledge of different types of disorders of endocrine, renal, GI, cardiac, pulmonary, hepatic, hematological, and neurological systems. The RN-Clinician II is cognizant of the psycho-social and financial impact that disease and hospitalization has on the patient and family and acts as an advocate when needed.

 

• Facilitates patient-centered rounding promoting collaboration and participation between family and health care providers.
• Assists Resident and Attending MDs to thoroughly review all cases on their respective medicine team, using resources available in Net Access, and other computer tools to locate and assess pertinent patient data.
• Collaborates with physicians, nursing staff, patient and family, and other disciplines to define plan of care with measurable clinical outcomes and anticipated discharge date.
• Monitors patient responses to treatments. Collaborates with patient/family, medical and nursing staff to adjust the plan of care as needed.
• Assists in interpreting findings with the team and shares findings with patient and family in a meaningful manner.
• Reviews records/processes to ensure compliance with core measure requirements and other quality measurements.
• Assists patient and family in navigating healthcare system for clinical needs.
• Fosters communication and collaboration with the healthcare team, the patient and family.
• Collaborates with patient and family, nursing personnel, and care coordination staff to assure needs are clearly understood and met (including psycho-social, financial, prescriptions, transportation or other factors that could contribute to delays in discharge).
• In collaboration with physician and patient/family assesses for referrals to other healthcare resources and assists with contacts.
• In collaboration with patient and family, facilitates post discharge care and follow up communication.
• • Maintains patient outcome as central focus. (Outcomes include excellent clinical management, appropriate patient education, follow-up care, and patient’s perception of satisfaction with services and care provided).
  • Develops and maintains collegial relationship with physicians, nursing leadership team and nursing staff through excellent communication skills, both verbal and written.
  • Identifies actual and potential problematic circumstances. Actively intervenes to prevent escalation and to attempt resolution.
  • Participates in leadership activities, such as committee responsibilities, JCAHO preparation, and assessment of the delivery of patient care.
