Join our team as an LPN Patient Navigator and play a pivotal role in coordinating all aspects of specific assigned clinical services. As an LPN Patient Navigator, you will serve as a liaison with other departments and interface with internal and external agencies, including diverse levels of medical and non-medical personnel, to ensure optimal clinical coordination. Your primary responsibility will be to assist patients in navigating clinical services, helping them overcome any barriers that may inhibit access to these services. Day Shift

University Medical Center - Orthopedics Clinic
 
Your Everyday  

Coordinates communication between patients, families, physicians, and other team members to ensure continuity of patient care and understanding of treatment plans. 

Facilitates appointments for consults and support services, tracking outcomes and collaborating with inpatient care management resources. 

Provides nursing care and guidance to patients from screening to survivorship, systematically assessing, planning, implementing, and evaluating care. 

Assists patients in understanding their diagnosis, treatment options, and available resources, initiating and documenting patient teaching based on assessment of needs. 

Acts as a liaison between patients and care providers, helping navigate the healthcare system and supporting patients during difficult decision-making periods. 

Supports the development and implementation of policies related to patient care services, updating or writing new policies as needed. 

Represents the organization at community educational events such as health fairs, screenings, symposiums, and lectures, serving as a resource for both community members and staff education. 

The Must-Haves 
 
Minimum: 

2 Years of professional nursing experience in a specialty area 

Current Licensed Practical Nurse through the Louisiana State Board of Practical Nurse Examiners.

Current American Heart Association BLS certification.
