In this role, you will be responsible for monitoring the health status of our clients, conducting assessments, and collaborating with a multidisciplinary team to address their needs. Your commitment to advocacy and quality care will be essential in fostering a supportive residential environment.

Key Responsibilities:

• Conduct and document monthly head-to-toe assessments of each residential client in their home.
• Maintain communication with agency management and the client’s support team, reporting and documenting all changes in client health status and needs.
• Ensure needs for staff supervision/delegation or specialized instruction are conducted and documented.
• Complete the Monthly Service Log detailing time spent performing Residential RN functions each month.
• Attend Individual Support Plan meetings and transition/discharge meetings for residential clients.
• Act as an advocate for persons served and complete other duties as assigned.

Education, Experience, and/or Credential Qualifications:

• Must be a currently registered, licensed registered nurse in the State of Missouri.

Additional Qualifications:

• Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.
• Current driver’s license, acceptable driving record, and current auto insurance.
• Must have and maintain current Missouri license as RN and maintain good standing.
• Must have current certification in CPR and First Aid.

Physical Requirements:

• ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.
• Sedentary work involves sitting most of the time but may involve walking or standing for brief periods.
