Presbyterian Medical Group seeks a Nurse Practitioner or Physician Assistant to join our Medicare Advantage Clinic in Albuquerque, NM. We're all about well-being, starting with yours. What We’re Seeking:

  • Nurse Practitioner or Physician Assistant with minimum of 2 years of Adult Geriatric experience
  • Have an active, non-probationary state medical license.
  • A motivated teacher, with patience and empathy for those with limited medical literacy
  • Focus on patient experience and ability to step in where needed, including but not limited to, blood draws, POCT, and education
  • Social, open-minded, diversity-literate, and approachable
  • Comfortable with a heightened degree of collaboration, including using our large exam-consult suites to foster a sense of comfort and empowerment for patients, their families, and their caregivers
  • Ability to deliver and render virtual health
  • Motivated to master the art and science of risk capture, quality improvement, clinical data management, taking advantage of clinical documentation and performance improvement opportunities
  • Graduate of a formal course in coding principles that includes knowledge of EDS and RAPS reporting
  • A great collaborator with Social Work, Community Health, PHP benefits, pharmacy and unwilling to let a patient fall through the cracks

 

Responsibilities

Summary:

Conduct comprehensive health risk assessments in person or via a virtual visit to identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit. Review and address preventative needs for the patients being seen. Educate and counsel patient and family on any conditions identified during assessment and screening procedures. Make recommendations and communicate findings to Primary Care Physicians through documentation of outcomes. Work will involve direct medical management as well as collaborating with the physicians, pharmacists, nurse case managers, care coordinators, behavioral health specialists and other consulting physicians and other healthcare professionals who comprise the patients care team.

 

Responsibilities:*Primary health care provider, emphasizing a focus on accurate diagnoses and documentation of risk adjusting (HCC diagnoses). *Identify and address recommended preventative and screening services through direct patient care.*Work with interdisciplinary team to ensure high quality patient care. *Order, conduct and interpret appropriate diagnostic, lab, and radiology services.*Involved in teaching and counseling individuals, families and groups.*Serve as health care resource, interdisciplinary consultant and patient advocate.*Performs other functions as required.

 

Credentials Required:

New Mexico Board Certified Nurse Practitioner. Masters degree in nursing. New Mexico license required. Three or more years experience in an inpatient or outpatient setting in family practice, internal medicine, or geriatrics. At least one year experience in caring for Medicare Advantage members required
