Provide Pathways services. Primary population is program-eligible pregnant women to ensure they have a medical home for prenatal and post-partum care, practice appropriate pregnancy self-care, get connected to needed social services to remove barriers to care, and learn basic maternal and baby care with the goal of achieving a full term, healthy birth weight birth. Provides education on topics including safe sleep practices, benefits of breastfeeding, and dangers of tobacco use. Services are home-based. May also serve high needs, low-income individuals who are not pregnant.

Preferred: Certified Community Health Worker. If no certification, will need to obtain within 12 months of hire. Required: High School diploma or GED, valid Ohio Driver's License and reliable transportation. Should have some experience in the human services field. Good oral and written communication skills, enjoy working with people, be a self-starter, have knowledge of community resources needed.
