Physical Therapist - Home Health
This is a full time position, working Monday-Friday 8:00am-5:00pm PST
Compensation Band: $68.00 to $72.00 per hour
The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. 

Key Responsibilities:

• Delivers physical therapy services in accordance with the physician’s plan of care, including assessing functional status, home environment, muscle strength, mobility, gait, and rehabilitation potential.
• Develops and directs physical therapy treatment plans, determines priority needs, and provides instruction on therapeutic appliances and exercises to patients and their families/caregivers.
• Initiates therapy programs and educates family members, caregivers, and other personnel on therapy goals and techniques; participates in case conferences to support coordinated care.
• Monitors and reports patient progress and any changes in condition to the physician; prepares and submits clinical and progress summaries in alignment with organizational policy.
• Trains patients in the use of prosthetic devices when appropriate.
• Supervises home health aides involved in patient rehabilitation plans.
• Identifies the need for additional home health services and makes referrals as appropriate.
• Participates in patient discharge planning and provides physical therapy consultation to families/caregivers as needed.
• Contributes to staff development through in-service education and peer consultations.

Position Qualifications:

• Possesses a degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.
• Licensed to practice as physical therapist within the State.
• Two years of appropriate experience as a physical therapist. Community/home health experience is preferred. 
• Possess and maintains current BLS Hands On CPR Certification.
