Job Details
The Clinical Pharmacist oversees the medication distribution system for their area of responsibility. In addition, the Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient.Job Description
Essential Functions
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Assures the appropriateness of drug therapy and compliance with hospital policies
- Oversees the distribution of medications throughout TJUH
- Assures the implementation of processes to support delivery of quality Pharmacy services
- Trains staff as assigned (e.g., interns, technicians, residents and new pharmacists)
- Precepts student pharmacists AND pharmacy residents. See Preceptor Development & Evaluation
Competencies (Knowledge, Skills, and Abilities Required):
- Identify and document sentinel/serious events, near misses, medication errors and adverse reactions in the electronic event reporting system
- Promote and assure compliance with the hospital formulary and policies and procedures defined by the P & T Committee
- Demonstrate expertise in application of pharmacokinetic principles to complex patient problems
- Demonstrate expertise in application of the literature to develop evidence-based recommendations in support of patient care.
- Provide quality patient care in accordance with standards of practice, demonstrating high level expertise, competency, knowledge and skills
Minimum Education and Experience Requirements:
Education:
- Doctor of Pharmacy
Experience:
- Completion of PGY2 Pharmacy Specialty Residency PLUS 2 years of relevant experience; OR Completion of PGY1 Pharmacy Residency PLUS 4 years of relevant experience; OR Completion of no residency PLUS 7 years of relevant experience
Minimum Certifications, Registration or License Requirements:
- Pharmacist License in the State of Pennsylvania
- Board Certification from the Board of Pharmacy Specialties preferred. In lieu of board certification through Board of Pharmacy Specialties, specialized certification in the area of clinical practice by the pharmacist may be deemed acceptable. To be evaluated on a case-by- case basis by the pharmacy supervisor.
