POSITION SUMMARY:

Under the direct supervision of the Director of Pharmacy, the Staff Pharmacist – MTM (Medication Therapy Management) will optimize medication regimens for APLA Health patients via comprehensive medication review (CMR) patient encounters.  MTM services will be provided as an extension of both APLA Health clinic and APLA Health pharmacy services. The MTM / Staff Pharmacist will perform their duties within the established standards of care, policies and procedures of APLA Health & Wellness.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Conduct comprehensive medication reviews (CMR’s). Key elements include medication reconciliation, patient education, and therapeutic recommendations or modifications in collaboration with providers.  
  • Screen for patients who are candidates for MTM services via provider referrals or routine chart reviews. Identify patients with complex medication situations that adversely impact their ability to take medication appropriately. Criteria include but are not limited to polypharmacy, chronic health conditions, high medications costs, significant drug-drug interactions, complex dosing schedules, high risk for adverse events, seeing multiple providers, and any other barriers that negatively impact appropriate medication uptake.  
  • Make outreaches to eligible patients via phone calls or digital communications. Schedule MTM visits with patients, whether in-person or through telehealth. 

 

In performing a CMR,  

  • Update patient’s list of active medications and allergies in electronic health records, including prescriptions, OTC, and supplements. 
  • Counsel and train patients on proper use of medications, devices, and supplies. 
  • Evaluate medication adherence. Assess barriers and formulate action plans where needed.  
  • Offer support and guidance to patients. Implement a significant change in medication utilization.  
  • Make therapeutic interventions under a collaborative practice agreement when appropriate. Communicate with providers about appropriate targeted interventions. 
  • Document clinical recommendations, responses to therapy, and patient care notes in a clear, concise, timely, professional manner. Encounters are documented to ensure proper billing of MTM services through various payers. 

 

Prepare reports to summarize success of MTM services as measured by  

  • Improved patient adherence 
  • Lowered medication costs 
  • Improved clinical outcomes  
  • Patient personal goals and satisfaction.  

 

Other duties may be assigned based on the needs of the clinic or pharmacy, including   

  • Support for clinic staff on clinical inquiries or various tasks surrounding drug distribution  
  • Support to APLA Health pharmacy as a staff pharmacist.
  • Conduct routine pharmacist duties including but not limited to medication dispensing, required checks to ensure safety and medication appropriateness, patient counseling, and other tasks essential to the practice of outpatient pharmacy, including pharmacy technician duties in the case of an absence or crisis.
  • Support of APLA Health Pharmacy patients in their utilization of pharmacy services. 
  • Provide pharmacy patients with an outstanding experience that promotes consistent usage of pharmacy
