Supervision: Reports to Vocational and Transition Services Coordinator/Regional Director

Work Year Requirements: Full-time, Year Round


Overview :   As a Job Coach/ Transition Facilitator for West Bay Collaborative, you will use your experience and expertise to assist our students and clients with disabilities to learn and accurately carry out job duties.This includes, but is not limited to, providing one-on-one training tailored to the needs of our students/clients. This role requires the Job Coach/ Transition Facilitator to collaborate with the Coordinator and other members of the Vocational and Transition Services team to develop plans, assist with goal setting,teach effective career and job skills/tasks, and support adolescents and adults within training sessions and job sites. Strong communication and organizational skills, along with experience in employment and career planning are important in this role. 


Responsibilities:

  • Review referral information and authorizations for pre-planning information.
  • Analyze job tasks and work environments to plan appropriate training, supervision and support services for clients placed in assessment or work development activities
  • Coordinate community assessment placements with school, family, and business partners.
  • Provide training and instruction that will enable clients to learn job tasks and integrate naturally into the business work routine.
  • Observe and record client performance and work behaviors and complete required assessment reports.
  • Liaison with business personnel regarding: support needs of clients placed; client’s work performance; and building natural supports.
  • Develop and maintain relationships with business partners.
  • Maintain ongoing communication with Vocational Services Coordinator regarding client evaluation and/or training process and progress.
  • Maintain ongoing communication with ORS Counselor, school personnel and family regarding progress of student/client.
  • Work with other Project staff in meeting the overall goals of the Project.
  • Submit all required reports in a timely manner.
  • Other tasks as assigned by Administrator, Supervisor or Designee Supervisor


Requirements

 Minimum Qualifications:

  • Associates degree, with relative work experience preferred; 
  • Experience working with and supporting adolescents and adults with disabilities improve job skills and achieve career goals.
  • Excellent communication skills and writing skills.  
  • Must be available to work school and non-school hours 
  • Safe driving record and be willing to transport students and clients in agency vehicles required.
