Full Time Home Health Float Pool position based in WI supporting the northern patient service markets. Areas of work based on assignment. Typical schedule is Monday through Friday 8:00am to 4:30pm with some variability based on departmental needs. Every 4th weekend rotation required.
Pay Range:
$35.50 - $53.25Major Responsibilities
- Complete scheduled patient visits and collaborate with the interdisciplinary team.
- Conduct comprehensive patient assessments and update plans of care as needed.
- Communicate treatment changes with physicians and the primary registered nurse.
- Monitor patient response to care and revise interventions accordingly.
- Coordinate scheduling and case conferences with primary nurse when appropriate.
- Assists with the discharge planning process according to agency standards and utilizes interagency/system and community resources to ensure continuity of care after discharge.
