Shady Grove Medical Center seeks to hire a Clinical Pharmacy Specialist who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.
As a Clinical Pharmacy Specialist in Psychiatry you will:
•Provide Pharmaceutical care by performing clinical, distributional, educational, informational, and operational functions.
•Serve as a resource for pharmaceutical care activities and act as an educator to the professional staff.
•Assist the clinical manager in designing the future direction of pharmacy services under the pharmaceutical care model through staff development and establishment of collaborative practice relationships with other members of the healthcare team.
•Act in conjunction with the clinical manager to assist with QI indicators and programs for the pharmacy utilizing information from recognized references such as ISMP and JCAHO; as well as assist with the institution of programs and reports to be utilized by hospital staff incorporating findings from QI programs. •Serve as the clinical pharmacy expert in their field of specialty (i.e. Critical Care, Emergency Medicine, or Antimicrobial Stewardship, Psychiatry and Transition of Care).
•Conduct medication group and discharge counseling
•Assist with admission and discharge medication reconciliation and streamline/ensure appropriate formulary use
•Improve medication adherence, reducing antipsychotic polypharmacy, and providing a patient-centered care through shared decision making
•Monitor for drug-related adverse events, reduce the overuse of psychotropic medications and provides comprehensive medication management through collaboration with the care team
Qualified candidates will possess:
• Graduate from an ACPE accredited pharmacy school, with PharmD degree required
• ASHP accredited PGY-2 Residency, or PGY-1 residency with Minimum of 3 years of clinical pharmacy experience in area of specialty required
• Registered Pharmacist with the Maryland Board of Pharmacy
Preferred (required within one year of employment): Board Certification in area of specialty required
• ACLS Certification preferred
• Member of American Society of Health-system Pharmacists (ASHP), and/or Maryland Society of Health-system Pharmacist (MSHP), or other relevant professional organization preferred
• Prior experience precepting students and residents preferred

Monday through Friday position

$105,664.42 - $158,496.00
