Job Summary
This is a full-time professional position. This position is responsible for coordinating nursing staff to ensure day-to-day continuity of care for all patients on service and ensure accountability of nursing care consistent with the agency’s mission statement and regulatory systems. This position supervises Transitions Community-based Hospice nurses plus day to day supervision of all interdisciplinary team members. The schedule for this position is Monday-Friday 8:00am-5:00pm; MBU On-Call Rotation; 1 major and 1 minor or 3 minor holidays per year.
Responsibilities what you do
- Coordinate interviewing/hiring of new clinical staff as assigned.
- Supervise and coordinate delivery of patient care for assigned patient care area on a 24-hour/day basis.
- Coordinate and problem-solve patient care issues with patients, families, physicians, agency staff and other health care providers.
- Ensure documentation of patient care is complete, timely, and accurate in accordance with TL processes, policies, procedures and regulatory requirements.
- Manage various personnel actions including, but not limited to hiring, orientation, performance appraisals, promotions and scheduling; and counsel and conduct formal discipline of clinical staff.
- Develop and implement processes to enhance clinical operations and practices for ongoing quality improvement.
- Case manage patients, as needed/requested.
- Oversee clinical compliance with all regulatory and accreditation needs of the agency.
- Review, update, and implement policies and procedures as appropriate, per agency protocol.
- Lead interdisciplinary group (IDG) meetings and case conferences.
- Oversee preparation of per diem staff calendar/scheduling.
- Identify annual competency and staff development needs, in conjunction with PI, Education, and Managers, ensuring all clinical staff are compliant with identified competencies.
- Attend seminars/workshops as appropriate.
- Covers Management Back-Up (MBU) as assigned.
- Covers at least one holiday per year as assigned.
- Professionally manages time, people, and technology.
- Perform other duties as assigned by supervisor.

Requirements what you need
- RN with current N.C. license.
- Two years of previous supervisory experience preferred.
- 5 years recent clinical experience with hospice, home health, or equivalent.
- Thorough understanding of Hospice clinical interventions and protocols.
- Knowledgeable about reimbursement sources, Medicare/Medicaid regulations, licensing laws and accreditation standards.
- Excellent written and verbal communication skills.
- Strong team building skills
- Strong leadership abilities. Excellent verbal and written skills.
- Competent in use of computer, able to learn and teach use of new software programs.
- Must have access to transportation.
- Requires travel to TL offices within NC.
- May be required to lift 30 lbs.
- Computer literacy with Windows programs. Ability to learn Netsmart programs.
- Current unrestricted NC driver’s license.
- Current professional license or certification.
- Current CPR certification.
