Founded in 2004, Brilliant Corners provides innovative housing and housing-related services to those transitioning from or at risk of homelessness or institutionalization. We develop, own, and manage multi-family supportive housing and licensed residential care homes. Additionally, we implement a broad array of scattered-site, one-on-one supportive housing, and clinical case management programs through partnerships with developmental services, homeless services, veteran services, and healthcare sectors.

PROGRAM SUMMARY
The Flexible Housing Subsidy Pool (FHSP) is a supportive housing rental subsidy program of the Los Angeles County Department of Health Services (DHS), along with other governmental partners and the Conrad N. Hilton Foundation. The goal of the FHSP is to secure quality affordable housing for DHS patients who are homeless. Brilliant Corners, the central coordinating community-based partner for FHSP, works to secure a broad range of housing options, including single-family homes, individual apartments, blocks of units, or entire buildings. Brilliant Corners identifies and secures units Countywide, provides move-in assistance and rental subsidy disbursements, coordinates with case managers, and assists with neighborhood relations.
WHAT IS A SENIOR HOUSING COORDINATOR?
With guidance from the FHSP Program Supervisor team, the Senior Housing Coordinator will provide tenant-based housing support and coordination, client unit assignments and Unit IDs, move-in coordination, reasonable accommodation request reviews/support, Salesforce auditing, review monthly rent roll reports, and housing retention services to participants of the Flexible Housing Subsidy Pool (FHSP). In addition, the Senior Housing Coordinator will support supervisor and team members as needed and may act as the point of contact when program supervisors are unavailable. The Senior Housing Coordinator will maintain a caseload of 50 clients and will work directly with tenants and ongoing case management to ensure that tenants successfully retain their housing over time. The Senior Housing Coordinator will ensure tenant well-being and unit habitability by conducting weekly, monthly, and quarterly home visits. The Senior Housing Coordinator will work creatively to prevent eviction and other housing-related issues with ICMS Providers and Property Management.
Responsibilities
- Collaboratively work with ICMS Provider(s) and Property Management
- Coordinate regular meetings with ICMS Team and Property Management team and take the lead on facilitating the meetings on behalf of Brilliant Corners.
- Coordinate move-ins
- Liaison with Intensive Case Management and Property Management
- Review and provide support for reasonable accommodations
- Audit Salesforce data
- Ensure tenant well-being and unit habitability through a regular schedule of unit inspections and wellness checks.
- Review monthly rent roll reports
- Support and coach housing coordinators with difficult cases
- Support with General Assistance request approvals
- Respond to housing and tenant-related emergencies during normal business hours
- Maintain a caseload of 50 FHSP clients.
- Become competent with the housing rights of people with disabilities under Section 504 of the Rehabilitation Act of 1973 and the Fair Housing Act, As Amended in 1988
- Assess the need for reasonable accommodations/unit modifications and ADA compliance, and assist program participants through the process of obtaining the request
- Submit and follow up on tenant maintenance requests
- Maintain thorough and accurate progress notes, files, and correspondences while maintaining the confidentiality of tenants, staff, and organizational information at all times and exercise appropriate boundaries with tenants
- Routinely make home visits and phone calls to program participants
- Attend agency staff meetings, training, department meetings, and case conferences Attend all agency staff meetings and training, as well as department meetings and case conferences
