Responsibilities

1.    Primarily responsible for providing an accurate, timely, and well communicated schedules for all patients upon referral through admission, and discharge from the agency.

2.    Actively participates in agency back office daily operations as assigned by the BOM.

3.    Demonstrates flexibility and use of critical thinking to address frequent changes that occur throughout the day requiring schedule adjustments as needed.

4.    Demonstrates that field scheduling assignments follow agency standard operating procedures (SOPs) and provides and maintains visit productivity for various levels of partners in the agency.

5.    Keeps up the Home Health staffing availability report by mid-morning daily to provide information to other key partners of the organization.

6.    Notifies the primary care physician ( as appropriate ) of any changes noted during care provision and secures new orders as needed.

7.    Other duties as assigned include, but are not limited to, intake, verifications, and documentation review.

 

Qualifications

 

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

  • Current, active, and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice
  • Minimum of 1 year of practival nursing experience
