Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness.  Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.

Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis.

Scope of Work:
Academic Pharmacy Fellow on Flourish Health Clinic

Key Responsibilities:
 

Flourish Health Clinical Service Delivery
a. Provide medication-related and clinical services including:

• Medication reconciliation and counseling

• Medication verification and dispensing

• Naloxone and fentanyl test strip distribution and education

• Point-of-care testing (e.g., glucose, A1C, lipid panels, rapid HIV/HCV, COVID/influenza)

• Preventative care services (e.g., administer screenings, immunizations)

• Chronic disease management and acute care services, including medication prescribing under collaborative practice agreements with physicians

• Developing and reviewing clinical treatment protocols and patient care workflows

• Safe medication disposal efforts

• Participate in behavioral health, harm reduction, and crisis intervention services, including initial assessments and referrals

• Participate in overdose response and provide naloxone as clinically indicated

• Ensure accurate documentation of all patient encounters and clinical services in the EMR

• Collaborate with interdisciplinary team members including medical providers, occupational therapists, physical therapists, social workers, counselors, and outreach teams in interdisciplinary case discussions and care planning

• Participate in training and mentorship of student learners on clinical care

• Assist in maintaining inventory and regulatory compliance for medication handling

• Support monitoring of clinical service metrics and patient outcomes

b. Harm Reduction & SUD Best Practices:

• Deliver patient-centered care grounded in harm reduction and trauma-informed practices

• Administer or supervise naloxone distribution, fentanyl test strip education, and safer use counseling

• Support the integration of evidence-based harm reduction models into mobile clinical workflows, such as screening and referral for MAT and behavioral health services

• Participate in training and mentorship of student learners on stigma reduction and culturally responsive care

• Assist with educating and implementing strategies for stigma reduction in clinical care

• Review current literature and program outcomes to support continuous improvement

• Collaborate on data tracking tools for monitoring harm reduction supply distribution, utilization, and patient outcomes

• Provide clinical feedback and recommendations during program evaluation and CQI processes
