Exciting Opportunity for a Full-time LCSW in Kingston, NY and Woodstock, NY Nuvance Health has multiple exciting opportunity for an experienced LCSW in Kingston and Woodstock, NY. The Clinical Care Coordinator known in the PC offices as a Behavioral Health Consultant is an integrated member of the Primary Care team and provides behavioral health services to the primary care patients. The schedule work hours are flexible within the Primary Care office hours Monday through Friday.Summary: Social Work Care Coordinators known as Behavioral Health Consultants provide clinical social work interventions which include counseling, bereavement, brief solution focused therapy and utilizes different modalities MI, CBT, DBT, mindfulness, supportive counseling etc. and crisis intervention evaluations as well as complete full biopsychosocial assessments and follow up sessions in compliance with CMS guidelines and bill for their services. The LCSW will assist in complex cases with patients with chronic medical illness to impact health behaviors. LCSW will work collaboratively with the PC providers to treat patients holistically and in one location with the support of the department of psychiatry. The LCSW also participates in Simulation Lab to promote SBIRT and MAT in the PC offices and assists in training medical students and residents.Requirement:
- Master's Level Degree of Social Works
- Minimum Experience: three to five years.
- Licensed Clinical Social Worker in NY and ability to obtain CT LCSW
Grade: S11
Salary Range:
$32.6400 - $65.0100 Hourly
