830a-5pCollaborates with Care Manager, Inpatient Care Team, Educare and other resources to assess patient needs upon discharge, give appropriate clinical and administrative education, and ensure that the patient and family understand discharge instructions. Meets with patient and family members to inform them of the care transitions – giving them the discharge plan, answering questions, and making follow up calls to the patient. Ensuring the patient and the family understand what to watch for, medications (purpose, side effects, how and when to take), follow up appointments – who, when and where, assess support system and equipment needs. Will document and assist with barriers in the support system and/or equipment needs. If the patient does not have a PCP, will refer to Pavilion Family Medicine and make appoint. If patient does have a PCP, schedule follow up appointment. Will assist with scheduling follow up appointments. Maintains accurate and timely documentation. Expedites the discharge process by preparing the patient discharge instructions to include making follow up visit appointments and med reconciliation. This nurse will obtain signatures on paperwork, meet all discharge criteria, arrange escort out of the hospital, and notify EMS. When time allows, will assist with admissions and transfers. Provide excellent customer service skills, be a Patient and family advocate, provide Patient and family education and may require assuming patient load. This nurse is responsible for carrying out the mission, vision, and quality commitment of Houston Healthcare.
Graduate from an approved School of Nursing.
Two (2) years nursing experience.
Current US state LPN license with authorization to practice in the state of GA. Current AHA/ARC BLS certification.
Must be able to utilize standard office equipment. Must be able to utilize diagnostic equipment as well as patient care equipment. Must be able to utilize personal protective equipment such as gloves, gown, mask, etc.
Successful candidates will be subject to criminal background and drug screens.
