Wage Range: $65,000-$75,000 annually, depending on experience
Position: Medical Social Worker
Service Area: Lifespark Corporate Office - St. Louis Park, MN 55416
Hours: Monday through Friday, 8am-5pm. Full-time, 1.0 FTE
The Medical Social Worker is an integral part of the interdisciplinary care team, providing comprehensive psychosocial support to patients, families, and caregivers. This role is essential in addressing psychosocial factors, mental health concerns, caregiver support, and access to community resources, directly impacting the effective treatment of medical conditions and the overall well-being of patients. The Medical Social Worker collaborates closely with clinical teams to ensure holistic, patient-centered care.

Position Functions & Responsibilities:
Care Coordination and Resource Assistance
-
Assist patients with psychosocial, financial, and practical concerns, including access to resources for activities of daily living (ADLs).

Facilitate access to in-home aides’ external caregiving support via agencies, meal delivery, and transportation services.

Connect patients and caregivers to funding and community programs for home modifications and fall prevention strategies.

Support transitions to alternative care settings (assisted living, memory care) when needed.

Assist with completing forms, documentation, and regulatory assessments for insurance products.

Help patients obtain assistive devices for vision and hearing needs.
Advance Care Planning

Support Advance Care Planning in the context of significant psychosocial challenges, such as impaired coping, family conflict, or lack of support during critical decisions or transitions

Assists clinical team to identify surrogate decision makers for clients without decisional capacity who lack a healthcare agents and / or power of attorney
Collaboration and Team Integration

Participate in interdisciplinary team rounds, interpreting social, emotional, and family issues for the care team.

Act as a liaison between medical staff and patients/families to minimize stress and improve coping skills.

Document assessments and care plans in the EMR and communicate findings with the care team.
