Responsibilities:
- Complete comprehensive nutritional assessments and individualized nutritional care plans for residents within time frames set by state regulations and facility policies.
- Perform Nutrition-Focused Physical Examinations and nutrition interviews
- Develop and/or evaluate enteral and parenteral nutrition prescriptions, making recommendations as needed.
- Evaluate the effectiveness of nutrition interventions and adjust as clinically indicated to support outcome goals.
- Monitor, evaluate, and document in the medical records of residents referred. Resident referred means residents who have been identified as being at risk of malnutrition or other nutritional problems, such as unintentional weight loss, poor food intake, or medical conditions that affect nutrient absorption.
- Participate in facility Nutrition at Risk (NAR) meetings to assess higher-risk individuals requiring regular interdisciplinary team evaluation.
- Participate in various meetings and committees as appointed.
Other Duties:
- Complete facility evaluation tools as requested and Continuous Quality Improvement (CQI) tools.
- Assist in problem identification and implementation of solutions for residents’ nutritional health and food service areas.
- Work with the Culinary Services Manager to ensure food service staff are demonstrating safe, sanitary practices and providing meals as prescribed.
- Provide nutrition-related in-services for culinary staff and other staff as required by state regulations or needed by the facility.
