Responsibilities

The Command Center Coordinator receives discharge/transfer information from internal/external sources and assists in the coordination of inbound patient Direct Admits and outbound transportation. Responsible for updating, communicating and printing Transfer Hub reports. Reconciles daily transport utilization and costs associated with transfer.

Qualifications

Education

o High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required Experienceo 2 years of professional experience in a healthcare setting required
