About the role

Under general supervision of the Associate Director – Clinical Services, this position provides psychosocial assessments and re-assessments, family consultation, supportive counseling, support groups, information and referral, and education for the target population.  Maintain a caseload of clients by monitoring individual plans of client self-care, coordination of services and ensuring follow-up on grant funded services for clients and their care recipients.

Required qualifications:

  • Master's Degree in Social Work from an accredited School of Social Work, or in Psychology, Sociology, Gerontology, Human Services or a related field from an accredited university.     
  • 4 years of case management/social work experience.
  • Candidates with Masters of Social Work degree from an accredited School of Social Work may apply with no prior paid work experience.
  • PALLIATIVE CARE: Minimum of two years social work experience and two years of hospice experience or acute hospital experience or ambulatory setting experience.

Preferred qualifications:

  • Fieldwork experience providing assessments and intervention at clients’ homes.
  • Social service case management experience working with families and older adults who are coping with adult onset cognitive impairments that include dementia, Parkinson’s disease, Huntington’s disease, Multiple Sclerosis, or related diagnosis.
  • PALLIATIVE CARE: Inpatient Palliative Care

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
