Job Duties

Western State Hospital is seeking a full time Counselor II Supervisor to provide supervision and performance management of social work staff on CMS-certified and JC-Accredited treatment units and to ensure that social work services are in concert with the mission of Western State Hospital. The Counselor II will develop and implement person-centered treatment and discharge plans in collaboration with consumers and other interdisciplinary team members; serve as a link between community agencies and others that support consumer recovery; assists the Clinical Social Work Director in developing SW Department standards of practice and developing and implementing policies and procedures; ensure the requirements of the performance planning and evaluation system are met, and evaluations are completed by established deadlines with proper documentation; performance issues are addressed and documented as they occur; and staff receive frequent, constructive feedback, including interim evaluations as appropriate.

The Counselor II Supervisor will also complete SW assessment within 7 days of admission; assess discharge needs in collaboration with other team members; Provides minimum required PSR interventions (generally 1- 2 hours/weekly); ensure discharge plans reflect a working knowledge of the forensic service delivery system and mental health law; meet with CSB liaisons to discuss discharge and recovery needs; social work progress notes reflect consumer, family/AR, and counseling/education; provide clinical documentation according to requirements and established time frames outlined by Joint Commission, CMS, and SW Department standards; and complete 10 hours of continuing education yearly (CME's or other formal training, workshops).

Minimum Qualifications

- MSW or MSSW required (graduation from a CSWE-accredited undergraduate or graduate social work program)
- Professional work experience with adults with serious mental illness
- Experience in discharge planning
