Schedule: Wednesday - Sunday; 3:00pm - 11:30pm (Days off Monday & Tuesday)
Rate of Pay: $23.00/Hourly (Entry Level) - $32.20 (Top) (Depending on experience)
Reports To: MMC Crisis Response Manager
(This position exists and is dependent on grant funding)
SUMMARY
Under the direction of the Crisis Response Manager, the client Navigator will provide survivor driven, and trauma informed services to those individuals/families within the community that are in need of Domestic Violence (DV) services, and who may be experiencing a DV crisis and/or situation. This includes inquiry of information about supportive services at the Marjaree Mason Center (MMC), in addition to providing the appropriate information and linkage of referrals, which may include the transportation of Domestic Violence victims from other Coordinated Entry System (CES) access sites. This position will complete a service needs assessments, including crisis assessments, and will provide supportive services to assist clients into the correct system of care if not MMC. Through assessment, the Navigator is responsible for diversion, crisis response, intervention, shelter referrals, safety planning, shelter/safe housing placement, and linking individuals/families with the appropriate destination for continued services.
This position will be responsible for the Intensive coordination and communication within the coordinated entry system process; and collaborate with internal and external entities in order to deliver more efficient use of resources. Navigators will be fully responsible for the enrollment data entry of all new intakes into the Safe House, in addition to the statistical and documentation entry as required; and responsible for the coordination and communication within the MMC Residential and Crisis Response team of all new client entries that are admitted into the Safe House. With the implementation of Navigators DV victims will have increased access points; and increased safe/confidential referrals to appropriate services within and outside MMC projects.
ESSENTIAL DUTIES AND RESPONSIBILITIES
• Serves as a Liaison between MMC and various service provider sites (as assigned), in order to ensure clients seeking DV services through alternate access points other than MMC, are received and assisted appropriately;
• Works with the Continuum of Care (CoC) CES Navigators and respond and support their efforts at Multi-Agency Access Point (MAPS) locations, other homeless shelters, and services providers to identify those DV homeless who have not sought services but are homeless;
• Works in conjunction with MMC Crisis Response and Residential teams;
• Rotates weekly attendance at existing MAPS locations throughout Fresno/Madera County to assist with Domestic Violence client referrals and provider service navigation through initial assessments;
• Assists and integrates programmatic components of Domestic Violence interventions to support other homeless housing;
• Collaborates with MMC Prevention and Education department to conduct community presentations as assigned;
• Establishes access points at the five (5) major hospitals within Fresno, Clovis, and Madera in order to expand points of entry especially for those fleeing Domestic Violence. Navigators will work in collaboration with the identified medical team, in order to effectively streamline DV referrals;
• Conducts assessments of service needs, in-person, over the phone or using video conferencing technology, including that of Crisis Assessments; and provide supportive services to assist those experiencing a DV crisis, assisting them into the correct system of care if not MMC;
• Makes informed decisions regarding client entry into the MMC safe house, based on client’s safety need;
• Conducts comprehensive safety planning for all individuals/families experiencing a DV safety concern/situation and provides education of the cycle of violence, and dangers of strangulation.
• Diverts or links individuals/families with the appropriate services of care, with the goal of determining the appropriate needs and interventions;
• Adheres to the intensive coordination and communication within the coordinated entry system process and collaborates with internal and external entities in order to deliver more efficient use of resources;
• Uses MMC or personal vehicle to provide/arrange transportation/pick-up of Domestic Violence victims from other public community sites or make home visitation. Travel will be between Fresno and Madera counties, including rural areas.
• Tracks mileage when using personal vehicle and submits mileage reimbursement form in a timely manner;
