Work Schedule: Varied work schedule. 40 hours per week.
The purpose of the PGY1 residency program is to develop a well-rounded pharmacist who is prepared to successfully practice in patient care positions, clinical informatics, leadership, and/or management positions. Pharmacists completing the PGY1 pharmacy residency program will be competent practitioners, skilled in evidenced-based patient medication management. They will understand how the integration of clinical, technical, and distributive functions is necessary to practice pharmacy successfully in an institutional setting. They will gain confidence with drug literature evaluation, pharmacy research, professional communication, medical informatics, as well as leadership and management skills.
The site will provide the knowledge and skills necessary through education, training, and experiences to provide pharmaceutical care in a variety of patient care settings, while understanding the involvement of informatics and technology. Upon successful completion of the program's curriculum and experiential training, the resident will demonstrate that he/she is competent and proficient in his/her ability to fulfill the required ASHP PGY1 program outcomes and to practice pharmacy at an advanced level.
This position will be responsible for adhering to policies, procedures, staffing, safety metrics and financial metrics. Education to healthcare professionals and patients is a requirement. Must maintain compliance with all Nebraska and federal regulations regarding medications.
Responsibilities:
Essential Job Functions
1. Clinical Core Competencies.
- Demonstrates competency with the established Clinical Programs approved by the Medical Staff.
2. Transfer of medication issues is communicated between the pharmacy team.
- ALL issues and pending transactions are communicated to staff for completion.
- ALL issues and pending transactions communicated to thee are completed.
3. Clinical Model adherence (Intervention documentation and observation.)
- Complete all assigned task based on assignment.
4. Drug formulary is preserved (Intervention documentation and non-formulary requests.)
- ALL non-formulary request communicated to prescribing physician with options.
5. Age Specific Criteria incorporated into clinical decision process.
- Screen all patients and adjust therapy options based on age of patient.
- Intervention documentation or completion of 1 Continuing Education (CE) program on age criteria.
6. Renal Dosage Adjustment (Adjustments noted within PharmNet.)
- Adjust all approved therapies on patients whose renal function dictates.
7. Pharmacokinetics Management ( (PK) sheets and intervention documentation.)
- Minimum 1/month: PK sheet correctly completed and entered into PK program.
8. Anticoagulation program (Intervention documentation).
- Complete monitoring and assessment of all anticoagulation patients.
- Appropriately doses all anticoagulation patients consulted.
9. Documentation of Interventions (Intervention documentation).
- Documented in a timely manner.
- Minimum 1/hr worked.
10. Operational Core Competencies.
- Effectively manages assigned areas based on established policies and procedures.
11. Oversee pharmacy technicians & interns to achieve FLOW in the medication process.
- Prioritize and maintain effective FLOW of the medication process.
12. Sterile Compounding Management.
- Pass Low Risk and Medium Risk Level Competency.
- Manage FLOW of products.
- Pass Chemotherapy Management Assessment and manage FLOW of products.
13. Central Pharmacy Management.
- Order verification complete and prompt based on assistance required.
