Assessor Reviewer - Field Based

Assessor reviewers conduct assessments for Medicaid beneficiaries with intellectual and developmental disabilities. Assessments occur in-person at the individual's home or in community settings unless the individual’s family cites health or safety concerns, and they request a virtual assessment.
Assessments should be submitted for quality review within 1-2 days of the assessment interview, and the assessment must be submitted to the client within 10 business days of the interview.

Essential Functions

• 65% Performs in-person asessments of persons with disabilities and virtual assessments as requested. Some contracts may require collaborating with health care providers and ancillary service providers to create or validate the appropriate treatment plan.
• 20% Documents assessment results for the individual member status or negotiates service plan as needed.
• 10% Provides ongoing education. May communicate with all health care providers to ensure appropriateness of care.
• 5% Performs miscellaneous duties as assigned.

Requirements

• Two or four-year degree in a health care or related field.
• Required to pass and maintain a SIS certification (Telligen will provide).
• Minimum of 2 years of experience working with persons with intellectual and developmental disabilities.
• Reliable transportation, reliable Internet connection, and proficiency in written and spoken English are essential.


• Experience in human services with the ID/DD Medicaid population is strongly preferred.
• Experience with assessments is a plus but not required.
