Summary: LPN performs a wide variety tasks which are broad in scope, and involve many complex and significant variables. The LPN functions in a task-oriented role under an RN’s direction. LPN works according to standard agency policies and procedures and standard practices of the profession, exercising independent judgment to implement risk plans and solve problems. LPN works with moderate supervision, referring to supervisor unusual or unprecedented situations. LPN maintains frequent contact with participants, agency staff, other service providers and the public for the purpose of exchanging information, medical coordination and solving problems.
Duties and Responsibilities:
1. Maintain compliance with all regulatory bodies
· Performs nursing duties and provides client nursing care in accordance with The Indiana Nurse Practice Act, MedicaidWaiver standards, ISO standards, Indiana State Department of Health standards, and Easterseals Arc of NE Indiana policies and procedures.
· Completes required data reviews and updates RN with any concerns
· Meets Balanced Scorecard Goals including billable time
2. Ensure services provided are individualized
· LPN will have client caseloads consisting of clients on Medicaid Waiver and in Group Homes.
