As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You’ll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients. You’ll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.
Location: Round and Travel within an assigned geography including Dialysis Clinics and Nephrology Practices.
Home Office, High Speed Internet and Reliable Transportation Required.
Position Details:
Complete Comprehensive Health Assessments (80%) of the time
Monday- Friday schedule with the ability to accommodate patient and nephrology partner’s availability. Must be flexible.
Holistic management of patient conditions, including assessment, diagnosis, treatment, and consultation.

Qualifications:
Minimum of two (2) years' experience as a Nurse Practitioner (NP) working in Value Based Care including documenting and coding from an MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high risk/chronically ill patients.
Current unrestricted NP license to practice in the state
Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
Passion for delivering care to patients with complex, chronic illnesses
Home Office, High Speed Internet and Reliable Transportation Required.
Preferred Qualifications:
Experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management
Spanish-speaking preferred in some markets
Experience in managing chronic diseases (CHF, diabetes, hypertension, diabetes)
Manage pop. Health (hospitalization)
