Position Summary:
The Elevated Community Health (ECH) Registered Nurse will promote, maintain, and improve individual and community health by supporting patients in adopting healthy behaviors. The Registered Nurse will provide safe, effective, and quality primary, preventive, and evidence-based health care to patients in a culturally competent manner, focusing on health promotion and disease prevention. The Registered Nurse is responsible for assisting medical providers and medical assistants in the efficient and orderly delivery of medical services and performing related duties as assigned by the Chief Medical Officer. The Registered Nurse acts as a resource to the medical assistants, front desk staff, health educators, and providers.
The Registered Nurse must demonstrate regard for the rights of all patients and provide respectful and considerate care focused on the patient's needs. The Registered Nurse will provide patients, families, and personnel with a safe and confidential environment.

Essential Functions:
- Deliver quality service, creating solutions that add value to a patient's health.
- Provides clinical support to medical assistants, community resource coordinators, front desk staff, and providers.
- Provide high-quality patient triage over the phone or in person.
- Provide case management for high-risk patients requiring additional support navigating the healthcare system.
