About the role

This position will be supporting the Sacred Heart Medical Center and Holy Family Hospital.

The Home Infusion Clinical Liaison position is responsible for creating and maintaining positive referral relationships in order to increase Home Infusion Referrals within assigned accounts.  This role includes working with discharge planners, facility staff, physicians, and other individuals to assess appropriateness home infusion, facilitate patient care transitions onto service and ensure there is appropriate documentation for ordered infusion therapy.  Consultative functions include, but are not limited to: providing consultation and informational presentations regarding home infusion therapy, and serving as a clinical resource and educator for ordering providers, facility staff, patients, families  on Home Infusion Therapy.

Required Qualifications:

  • Education sufficient for clinical licensure/registration for discipline (e.g., LPN LVN, Registered Dietitian).  
  • Upon hire: Current state professional license or certification appropriate to discipline.
  • 3 years experience in clinical discipline.

Preferred Qualifications:    

  • Bachelor's Degree in Applicable clinical discipline (e.g., Respiratory Therapy).    
  • Home Infusion Liaison, marketing or sales experience.
  • Health care industry experience.
  • Documented prior successful sales account medical management experience.
  • Epic EMR experience.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
