Inpatient Care ManagementOverview of Position:
The care management associate works under the direction of department leadership to provide administrative and patient facing support for care management processes. The associate can perform tasks identified by the care management team for discharge planning and care coordination support. This position does not provide any support that requires professional intervention. Provides back up to the department assistant by handling correspondence, maintaining files, acting as a receptionist, screening telephone calls, opening and routing mail to appropriate personnel within department and compiles departmental monthly reports. Performs scheduling of staff and payroll for department.ESSENTIAL DUTIES
1. Completes tasks related to discharge planning such as transfer of patients to post-acute facilities including:
a. Communication with the patient and family regarding choice of nursing facilities.
b. Communication with facility related to bed availability, including use of care management facility communication within EPIC.
c. Sharing of internal referral information electronically or via fax.
d. Assuring transfer of documentation/ medical record complete.
e. Arranging transportation, confirmation of arrangements, and communication with families regarding transfer arrangements.
f. Maintaining and managing awareness of post-acute settings, such as skilled nursing facilities, home care agencies, infusion services, durable medical equipment services, hospice, acute rehab, and long-term care facilities.
g. Demonstrating independence and proactive initiation of duties related to transfer of patients described above.
2. Provides patient and/or family with second important message letters, as directed by care management staff.
3. Secures requested information for the healthcare team, patient, and/or family. This could include prescription availability, insurance requirements, and community resources.
4. Supports the inpatient care management staff in tasks related to care coordination and/or discharge planning.
5. Assists with data collection.
6. Keeps team members updated with progress of assigned tasks. Escalates issues to care manager, social worker, or leadership staff in timely manner.
7. Maintains and adjusts department schedule to enhance the performance of the department.
8. Documents accurately and thoroughly all transactions during the shift, according to department policy.
9. Independently prepares reports and drafts correspondence involving synthesizing information, writing text and preparing final communications for distribution with a 98% error free result.
10. Completes and processes necessary paperwork according to procedures with a 98% accuracy rate. This includes preparing/distributing reports, depart goals/report card, correspondence, memoranda, and meeting agendas/minutes. .
11. Maintains an effective system for storing, filing and retrieving correspondence and reports to enhance efficiency.
12. Demonstrates proficiency and maintains an understanding of tools and resources used to complete work in a timely manner.
13. Independently prioritizes work to complete assignments within established time frames.
14. Performs background investigation into problem situations and supplies information to other staff and manager for further action. Follow-up and tracks problem until resolution has occurred.
15. Identifies opportunities to automate or streamline department or job-specific processes or procedures.
16. Assist with customer service issues for all department staff.
17. Verifies and submits payroll accurately and timely.
QUALIFICATIONS
1. Associates degree in a health-related field or Social Worker Assistant preferred.
2. Prior experience of 2 years in a support-related position required.
3. One year of related work in a healthcare environment preferred.
4. Sound knowledge of basic medical terminology preferred.
