Intake Assessment Representative - Patient Access

The Intake Specialist serves as the main access point for Home Health Services. This position is responsible for initiating and coordinating Home Health referrals, gathering all necessary referral and payor information and then inputting into EMR, verifying patient information and initial HH eligibility with patient and referral source, and coordinating and scheduling date for HH admission. This position ensures that information obtained at time of referral is accurate and complete. This position represents Home Health to customers and patients effectively promotes Home Services and products.

Required Qualifications:

• Coursework/Training: Educational coursework in medical terminology. Or equivalent educ/experience

• upon hire: In compliance with the Providence Health and Services-OR Human Resources policy #302 Background Checks, a criminal background check is required for this position, based on assigned duties. 

• 2 years Experience in customer service including demonstrated computer proficiency 

Preferred Qualifications:

• Bachelor’s Degree

• 1 year Experience in a clinical setting or home services.
