Position Title: Substance Use Clinician (CCAPP)

Program/Dept: SUTS OP

Reports to: Program Manager

Classification: Full-Time, Exempt 

Salary Range: $66,412+ DOEE + Full Benefits Package

Position Description: This position must be available to respond to Substance Use Disorders treatment including client intakes, individual and group counseling, treatment assessment and planning, and case management as needed.

 

Essential Duties & Responsibilities:

Provide direct counseling services, including individual and group counseling, treatment assessment and planning, and case management as needed.

oMonitor client progress and adherence to treatment plan; maintain client files and case documentation in accordance with DMC related policies.

oEstablish and maintain working relationships with community resources and agencies.oUtilize/make referrals to assist client in plans for education, vocational training, employment, housing, transportation, medical, mental health, etc

Maintains excellent client records and complete all agency required reports and paperwork.oComplies thoroughly with Santa Clara Substance Use Treatment and DHCS contract documentation and auditing standards and requirements in a timely manner.

Participate in group and individual supervision.

May train peers as needed and requested by management.

Perform other related duties, responsibilities and special projects as assigned.

 

Requirements, Qualifications, Skills & Abilities:

•Must be registered with CCAPP as CADC or LAADC.

•Bachelor’s Degree in related field preferred.

•Associate degree or equivalent amount of related experience.

•Excellent oral & written communication skills; strong interpersonal skills.

•Knowledge of Database software; Spreadsheet software and Word Processing software and overall excellent computer skills are required.

 •Knowledge of group development/dynamics is required.

•Demonstrated ability to work independently with initiative, using good judgment, and available resources.

•Demonstrated ability to organize workload, maintain multiple priorities, and sustain productivity and accuracy.

•Reliable and consistent attendance, punctuality and coverage of responsibility and commitments when absent is required.

•Bilingual in Santa Clara County Threshold Language strongly preferred.

•Knowledge of and relationships with the South Bay youth services community is preferred.

 

Physical, Environmental and Mental Requirements:

•Physical: Occasionally required to push/pull objects up to 50 lbs., and to lift/carry objects up to 25lbs. Frequently required to perform moderately difficult manipulative tasks such as typing, writing. Must be able to walk, stand, sit for extended periods.

•Sensory: Frequently required to read documents, written reports, and plans. Must be able to distinguish normal sounds with some background noise, as in answering the phone, interacting with residents and staff, etc. Must be able to speak clearly and understand/be understood using the English language.

•Cognitive: Frequently required to concentrate on moderate detail with constant interruption. Must
