KEY RESPONSIBILITIES:

·        Responsible for taking and processing Palliative Care referrals.

·        Provide support to the Palliative Care Nurse Practitioner and Medical Director.

·        Will schedule visits to support goals of care as ordered by MD.

·        Coordinate and communicate patient care goals between all providers.

·        Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.

·        Ensure communication of assessment and reassessment findings to attending physician.

·        Complete all documentation accurately, timely, and in accordance with company policy and procedure.

·        Inform palliative team of all new admissions and discharges.

·        Coordinate referrals utilizing the referral process.

·        Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.

·        Run billing report and address validation holds/ billing errors
