Job Description:

JOB SUMMARY

  • This employee carries a caseload of over 100 consumers receiving meals through the Office of Aging.

  • This involves conducting annual assessments in person at consumer's residence, home visits at 6 months, and at least 2 other contacts throughout the year.

  • Many consumers rely on the employee for resources and guidance, especially during gaps and changes in their informal supports.

  • Compliance with detailed documentation requirements is critical.

REPORTING RELATIONSHIPS

  • This employee reports to an Aging Care Management Supervisor.

  • Daily contact is maintained and supervisory meetings held as needed for the purpose of discussing ongoing assignments, program and personnel matters, and developing long-range plans.

  • Performance is also evaluated indirectly, through observation of his/her interactions with consumers and others, reviewing document in SAMS and correspondence with consumers.

ESSENTIAL JOB FUNCTIONS

  • Works with consumer and family to assess what help is needed, provide information and monitor progress to prevent a gap in consumer's services. Maintains contact with consumers as required by PA Dept of Aging directives. Refers complex problems to supervisor.  60%

  • Completes required paperwork and data entry, including extensive case documentation accurately and in a timely manner.  30%

  • In the course of performing job duties, employee will be required to provide own transportation to consumers homes, meetings and other job-related appointments and access those locations without special accommodations.

OTHER SPECIFIC TASKS OR DUTIES

  • Assists Care Managers in duties related to care management, protective services and other agency services which will benefit the consumer, to include identifying alternative service resources, and completing forms and other documentation.  10%

  • Attends all mandated training sponsored by the Pennsylvania Dept of Aging relative to community-based services and ot-her relevant enrichment sessions.

MINIMUM QUALIFICATIONS 

  • One year Aging Case Aide 1, OR

  • 2-years public contact work including 1 year in a Human Service program, OR any equivalent combination of experience and training.

KNOWLEDGE, SKILLS AND ABILITIES

  • Current knowledge of HIPAA regulations and their interpretation is maintained and adhered to.

  • Give and receive information and feedback in order to provide a more complete result.

  • Demonstrate respect for and awareness of others, good judgment in responding to conflict, and flexibility to adapt to changes in one's work environment. 

  • Understand and anticipate customer needs.

REQUIRED LICENSES/CERTIFICATIONS/CLEARANCES

  • Acceptable Pre-Employment Criminal Background Check

  • Acceptable Pre-Employment Drug Screening

  • Acceptable Driver History in accordance with County Policy

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

  • This job is not sedentary, carrying or lifting rolling laptop case weight, approximate weight is 6 lbs.

  • Employee may be required to access multiple levels in dwellings without elevator access in order to access consumers.
