Description
AMHC is excited about our new Veteran Care Coordinator position in the Caribou area. The Veteran Care Coordinator is responsible for facilitating and implementing outreach activities in the geographical area. The Veteran Care Coordinator will provide triage, support, advocacy, and connection around clients' identified social determinants of health and integrated coordination of care, based on the individual needs of the veteran or active-duty service member. The chosen candidate will be expected to demonstrate the core values of AMHC by showing Integrity, Compassion, Excellence, Service, Collaboration, and Respect to co-workers, clients, and community members.
Qualifications:
- MUST be a veteran in the U.S. military
- An associate degree is required
- Must have a valid driver's license and reliable transportation.
Responsibilities & Expectations:
- Respectfully and Compassionately support military-related individuals in receiving services.
- Participate in the Collaboration of discharge planning and continued care for individuals in or related to the Military.
- Coordinate outreach/peer activities maintaining the agency's integrity and people served.
- Uphold AMHC's Excellence by demonstrating veteran-based values and removing barriers in Service.
Schedule:
- This is a 40-hour-per-week non-exempt position
