About the role

Description

The Homelessness Resolution Connections Case Manager (HRCCM) is a full-time, non-exempt position of Denver Rescue Mission (DRM). HRCCM is responsible for researching, vetting, developing, and maintaining community connections for our Homeless Resolution Team (HRT) staff as well as providing case management and performing contract duties as assigned. This position will acquire and maintain knowledge and understanding of homelessness resolution strategies and rehousing systems translating these into accessible and usable resources for HRT staff. The HRCCM will ensure that clarity and cohesion exist between HRT staff and connections to community resources, facilitating the movement of clients through our systems towards housing stability.


Community Provider Relationships

  • Develop and maintain relationships with resource providers in the Government, Medical, Mental Health, Housing, Benefits Acquisition, Employment, Social, Legal, Disability, and Veterans fields, etc., to aid clients towards housing stability.
  • Ensure that clarity and cohesion exist between HRT and its connection to community partnerships.
  • Serve as the “contact/point person” for community providers who are onsite regularly and during special engagements at all Denver DRM locations.
  • Ensure onsite community provider services run smoothly, questions are answered, and issues are resolved efficiently and effectively.

Community Resources

  • Research and pursue resources to meet the needs and address barriers to housing for clients utilizing DRM services including Housing Resources and Landlord recruitment.
  • Maintain DRM database of vetted resources for accuracy and usability by staff and clients to ensure DRM’s case planning efforts reflect a real-time understanding of resources that aid in exiting homelessness and supporting housing stability.
  • Manage clear and consistent communication with DRM employees about community resources, including notifications of changes and updates.

Case Management

  • Develop thorough understanding of the 22 Things case planning tool and its application across DRM’s continuum of care. 
  • Work with individuals who are experiencing homelessness, addiction, mental illness, and a myriad of other issues related to homelessness and housing instability.
  • Develop and manage case plans with clients utilizing the 22 Things with the ability to identify action steps necessary to achieve goals.
  • Monitor client progress through scheduled follow up meetings, particularly for those utilizing housing vouchers and/or needing special accommodation.
  • Maintain professional best practices in areas of document storage, confidentiality and professional ethics when dealing with clients and client information.
  • Meet in person with guests and participants to support Bridge, Pathway Home, New Life Program and Next Step, Complex Case Managers, Rapid Resolution, and other assigned programs to provide assessments, assignments, and accountabilities to guests and participants.

Administration & Compliance Management

  • Maintain knowledge of legal and funding requirements for contract work and memorandum of understanding Memorandum of Understanding (MOUs).
  • Ensure accuracy and compliance with all contract documentation and reporting requirements.
  • Create, format, edit, and follow detailed workflows incorporating multiple databases, reports, and spreadsheets.
  • Produce and maintain data spreadsheets and reports documenting HRT expenses and outcomes to ensure alignment with DRM strategy and standards.

Homelessness Resolution Training & Development

  • Assist in the development, delivery, and scheduling of training for staff by both internal and external partners to better understand and utilize HRT resources and community connections.
  • Communicate with the public through speaking arrangements, community meetings, interviews, etc.

Work Environment

  • This job operates in a professional office setting as well as in DRM’s shelter and transitional housing facilities. This role routinely travels to and from facilities and properties, requiring the use of mobile devices for documentation and communication. The position requires in person one-on-one interactions with clients who are experiencing homelessness or are recently housed, both on property and off.
  • Performs other related duties as assigned.

Requirements

  • Bachelor’s degree in human services or related field.
  • At least one year of experience directly engaging (case management preferred) with individuals experiencing homelessness and/or housing instability.
