Adventist Healthcare seeks to hire an experienced Manager of Care Navigation who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing.

As the RN or SW Manager of Outpatient Care Navigation, you will:
•Lead team with primary function of connecting Adventist HealthCare patients, who are at high risk for readmission, to reliable and supportive community resources
•Lead readmission reduction efforts through root cause analysis and implementation of systems and processes to address identified risks and drive optimal performance of the department
•Lead team to collaborate with the acute care teams, post-acute care teams, and physician practices to ensure patients are navigated to the most appropriate level of care in outpatient setting
•Monitor team’s performance for outcome measures and process measures
•Lead delivery of care provided by Care Navigation team to ensure that patients’ needs are addressed, and that documentation meets standards
•Ensure team member’s active participation in Interdisciplinary Rounds and other patient care conferences
•Oversee and evaluate the performance of the Care Navigation team

Qualifications include:
•Bachelor of Science in Nursing required, Master’s degree preferred
OR Master's degree in Social Work required
•Maryland RN or Maryland Social Work Licensure (LMSW/LCSW/LCSW-C)
•Active American Heart Association Basic Life Support (BLS) certification required
•Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance as directed by AHC
•3–5 years experience in patient navigation, discharge planning, or care coordination; At least one year of prior leadership experience required

Work Schedule:

Full-time, Monday through Friday; position oversees team at Shady Grove Medical Center and White Oak Medical Center which requires travel between the sites as needed. Additionally, there are meetings in the community which require attendance.
