The Opportunity:

Licensed Professional Counselor, Licensed Clinical Social Worker (Oregon Board of Licensed Social Workers) or Licensed Psychologist (Oregon Board of Psychologist), preferred.  Clinical Social Work Associate (CSWA), Professional Counselor Associate (PCA), and other QMHPs pursuing licensure are also qualified to apply.

The Student Health Center program (SHC) is a unit within Multnomah County’s Community Health Center. The program provides primary care health care in 9 SHCs in coordination with five school districts: David Douglas, Parkrose, Centennial, Reynolds, and Portland Public Schools, and the Child and Adolescent Mental Health Program of Multnomah County Department of Mental Health and Addictions.

SHC is a powerful tool for achieving health equity among children and adolescents who unjustly experience disparities in outcomes simply because of their race, ethnicity, or family income. It’s also a commonsense model: place critically needed services like medical, behavioral, and dental care directly in schools so that all young people, no matter their zip code, or ability to pay, have equal opportunities to learn and grow.

The purpose of the SHC Behavioral Health Provider is to improve population health outcomes by providing behavioral health services to patients 5-18 years of age as a usual part of primary care. This position will function as a clinical member of the primary care home team, to work together with patients and families, to provide patient-centered care.

As the Behavioral Health Provider you will:

  • Provide behavioral health assessment, diagnosis, and brief evidence-based behavioral interventions usually lasting from 1-6 visits for patients of SHC with behavioral health concerns. 

  • Provide continuity care. Care plans may include talking or meeting with family or support members, or school staff and other community behavioral health partners. 

  • Formulate diagnostic and treatment recommendations and present findings to treatment teams. 

  • Utilize the program suicide risk patient protocol, which includes assessment, follow up, and care coordination to ensure patient safety and support. 

  • Participate in group medical appointments (shared medical appointments and drop-in group medical appointments) as requested by team members. 

  • Provide care via in person and telemed modalities. 

  • Have the opportunity to participate in professional development such as training, conferences, and program development activities.
